- Description: Surgeon Kwak Chan-yee of YourTime Plastic Surgery explains the techniques and realistic limits of forehead reduction, from scalp release to bone contouring and forehead lift.

Hello, I'm Kwak Chan-yee from YourTime Plastic Surgery.

"My forehead is huge — I really want to reduce it a lot."

"Can't you take off more? Is this really the best you can do?"

These are things I hear fairly often during forehead reduction consultations.

Wanting to reduce the forehead as much as possible — this is something both patients and surgeons genuinely want, and it's something I think about carefully.

There is a limit to how much can be removed. If too much skin is excised and the wound cannot close properly, the consequences can be serious. So I always explain that there is an appropriate range.

Because I have also presented on this topic at conferences, colleagues occasionally reach out to ask me about it.

So I'd like to explain what I tell patients and how the procedure actually works — as clearly and simply as possible, as always.

\=======================================================

1\. The most important factor is how much the scalp can stretch — and there are definite limits.

Stand in front of a mirror, press firmly on the top of your head, and try pushing the scalp forward and backward. Notice how much it moves.

Generally speaking, East Asians — and particularly males among East Asians — tend to have firmer, less mobile scalp, typically moving about 1 cm or so.

People whose scalp moves more have greater potential for reduction, but for those who don't, even after all the techniques described below, it is difficult to exceed a skin excision of 2–2.5 cm in East Asian patients.

Here is a 2025 Brazilian study I'd like to reference. Even among Hispanic patients, whose skin tends to be more elastic, the average reduction was 2.29 cm; for Black patients it was up to 2.52 cm. The general consensus from other Western literature is that the maximum is around 3 cm.

If a patient wishes to go beyond this, I also suggest considering hair transplantation, or non-surgical alternatives such as a change in hairstyle, scalp micropigmentation (SMP), or a hairpiece.

2\. Complete release of retaining ligaments, including temporal lift

![How to Reduce a Large Forehead Significantly (Feat. M-Shaped Hairline Recession) — 3](https://storage.ghost.io/c/a3/af/a3af241d-9135-4bef-8594-54b909eec4e4/content/images/2026/10/n2g_1792690294720_s7kewehzl98.png)

This is an illustration I have referenced several times — it appears in Part 1 of the Forehead Lift FAQ and relates to the temporal lift. Unless the retaining ligaments that firmly anchor the forehead-scalp complex to the skull are completely released, the forehead simply will not move. A thorough and clean release is the single most important step in achieving maximum reduction.

3\. For patients who are candidates for a forehead lift — performing a forehead lift simultaneously

For patients who still want more reduction after the above steps, I evaluate whether they are candidates for a forehead lift.

![How to Reduce a Large Forehead Significantly (Feat. M-Shaped Hairline Recession) — 4](https://storage.ghost.io/c/a3/af/a3af241d-9135-4bef-8594-54b909eec4e4/content/images/2026/10/n2g_1792690297511_7er9k8mq98m.png)

When the scalp is advanced downward to create the new hairline, if the scalp does not descend far enough, another option is to lift the forehead upward to meet it.

Of course, it is important to carefully assess whether the resulting eyebrow position and double eyelid line remain harmonious when the forehead is lifted, and whether the patient is a suitable candidate for a forehead lift. Combining a forehead lift with hairline lowering can allow an additional reduction of approximately 0.5–1 cm.

4\. For patients with significant frontal bone prominence (commonly called a "boss forehead") — considering bone contouring

![How to Reduce a Large Forehead Significantly (Feat. M-Shaped Hairline Recession) — 5](https://storage.ghost.io/c/a3/af/a3af241d-9135-4bef-8594-54b909eec4e4/content/images/2026/10/n2g_1792690300183_q49894wf3k.png)

![How to Reduce a Large Forehead Significantly (Feat. M-Shaped Hairline Recession) — 6](https://storage.ghost.io/c/a3/af/a3af241d-9135-4bef-8594-54b909eec4e4/content/images/2026/10/n2g_1792690302599_2scnt73to81.png)

Some patients have noticeable frontal bone protrusion — what is commonly referred to as a boss forehead. In these cases, shaving down the bone to reduce its height allows for greater overall forehead reduction.

![How to Reduce a Large Forehead Significantly (Feat. M-Shaped Hairline Recession) — 7](https://storage.ghost.io/c/a3/af/a3af241d-9135-4bef-8594-54b909eec4e4/content/images/2026/10/n2g_1792690305450_rl4057xxx2h.png)

I previously mentioned that patients who have undergone facial contouring surgery often experience skin laxity afterward and may want a facelift — and the principle here is the same.

By partially shaving down the bone and lowering its profile, the volume decreases, which modestly increases the length that can be reduced. In addition, the angle at which the forehead is exposed when viewed from the front changes, so even the same amount of reduction can appear more dramatic.

Naturally, bone contouring requires CT imaging beforehand to comprehensively evaluate bone thickness and the position of the frontal sinus before any decision is made. The patient's own wishes regarding whether to address the bony protrusion must also be taken into account.

5\. Periosteal scoring (incision) and sub-galea undermining of the scalp

If the scalp still does not stretch sufficiently, scoring the periosteum to gain additional length is another option — however, the indications are limited and there are associated risks, so it is enough to simply be aware that this technique exists.

Personally, I prefer to take a conservative approach with scalp excision and perform any additional trimming afterward if needed. I explain to patients that the initial design sets a target amount of excision, but that I will remove as much additional tissue as safely possible during surgery, and I do perform incremental excision at the end of the procedure.

This approach results in a safer operation overall.

\=======================================================

When it comes to forehead reduction, surgeons want to achieve as much reduction as possible — just as patients do.

But there are limits to how much can be removed.

The principle of "moderation in all things" always applies.

Because going too far carries real risks,

I encourage patients to determine through thorough consultation exactly how much reduction is appropriate for them before proceeding with surgery.
